You are invited to participate in our survey looking at health trends in the God Land community. You will be asked to complete a survey that asks questions about your medical and oral health. It will take approximately 5 to 10 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. It will be great if you can complete every question, however, if you feel uncomfortable answering any question or are unable to answer, you can leave the answer blank. You can also choose to save and continue the survey at a later time or end the survey at any point.
Your survey responses will be kept strictly confidential and data from this research will be reported only in the aggregate. Your personal information will be coded and will remain confidential.
